Project Goal
Assist with statewide implementation of HB 885 on permanency planning, blended funding strategies, and service delivery restructuring. Assist with the implementation of independent case management and family support models.

Project Summary
This project aimed to effect statewide systems change in order to support children with developmental disabilities (DD) and their families. Four local family-support collaboratives were to focus on services to children with DD. Key activities were to include assisting with statewide implementation of HB 885 on permanency planning, blended funding strategies, and service delivery restructuring. Additionally, the project aimed to facilitate implementation of independent case management and family support models; training and technical assistance; public awareness; and case review.
