Texas Department of Aging — Support for Individuals with Disabilities Who Are Aging

Project Goal
Establish collaborative work groups composed of area agency on aging staff and service providers, agencies that serve individuals with developmental disabilities (DD), and stakeholders from the public and private sectors.

Project Summary
The project aimed to establish collaborative work groups composed of area agency on aging staff and service providers, agencies that serve individuals with DD, and stakeholders from the public and private sectors. The desired result was a well-integrated service system for use by people with DD who are aging and their caregivers. The service system would be easily accessible from multiple entry points and users would be able to receive assistance, including personal futures planning, from personnel who are knowledgeable about and responsive to their needs.
