Transition Medicine Program: Expansion Project

Impact

    • Transition social workers provided 550 community consultations to patients, families, case managers, community agencies and health care providers across 30 Texas counties
    • 75 healthcare workers participated in delivery health care to adolescent and young adults with a chronic childhood condition
    • Over 24 presentations were provided by social workers and/or lead doctors to local community and physician groups
    • Over 300 adolescents and young adults with a significant chronic childhood illnesses or developmental disabilities (DD) established a medical home with the Baylor Transition Medicine Clinic

Project Goal
Train health care workers on how to care for people with chronic illnesses or DD. Provide outreach to community health care providers and families of children with DD.

Project Summary
Baylor College of Medicine (BCM) provided social worker support to adolescents and young adults with a chronic childhood condition or DD and their families. Medical professionals assisted youths and their families in identifying barriers to the transition from pediatric to adult care, and helping them solve those barriers by directing them to resources available in their area. Additionally, families were provided with case coordination if needed. BCM offered a medical home to those who could benefit from ongoing case management and coordination.

Adult health care students benefitted from increased exposure to the medical care of patients with a chronic childhood condition and worked alongside social workers to learn the different barriers and challenges these groups of patients face
