Texas Health and Human Services Commission — Development and Implementation of Statewide Permanency Planning Efforts

Project Goal
Support the monitoring of interagency permanency planning efforts within residential facilities in Texas to provide opportunities for children with developmental disabilities (DD) to live with families.

Project Summary
This project was created to support the monitoring of interagency permanency planning efforts within residential facilities in Texas, to provide opportunities for children with DD to live with families. It would allow the Health and Human Services Commission to develop an automated means of monitoring electronic reports submitted by providers so they could determine which ones needed individual attention. The project also aimed to train state surveyors in order to ensure that the quality of permanency planning would be evaluated appropriately during institutional surveys.
