Imagine Art — Peer Support in Communities 

Project Summary
This project supported individuals with developmental disabilities (DD) in creating person-centered plans so that they can become more independent. Through an art-based curriculum, participants expressed the ability to produce tangible expressions through mini-books, works of arts, songs, and theater. The training resulted in: transformation through creative personal story-telling using the arts (understanding of self); awareness of best practices for independent living; knowledge of long-term care options, waivers, and other government systems that serve people with DD; and, engagement in current activities that support systems redesign of home and community-based services and other long-term care systems.
