Establish a Transition Center and personal support network that will guide and provide support to families of youth with intellectual and developmental disabilities (IDD) transitioning into adulthood.
The transition period, the time between the end of public-school services and adulthood, can be difficult for families of children with IDD. Many families either lack access to services and supports for their children entering into adulthood or rely on a patchwork of supports that can be unreliable or include long waiting lists.
Texas Parent to Parent is providing comprehensive transition training, one-on-one guidance, and group support to parents of transitioning children. Through the project, parents receive information and assistance to develop plans for their children that focus on the components of a good life: loving friends and family, a home of one’s own, financial security, participation in decision making, and the ability to contribute one’s unique gifts. Lessons learned through the first two years of the project will be used to help partner organizations replicate the project in other areas.
